By using this website or submitting a support request, you acknowledge that Clarity Care Coordination provides non-medical services and does not replace physicians, nurses, home health agencies, emergency services, licensed transportation providers, social workers, attorneys, financial advisors, or other regulated professionals.

Service availability, response times, and applicable fees depend on the client's location and requested service. Submission of a request does not guarantee service acceptance.
