Scope of services. Clarity Care Coordination provides non-medical support and coordination. Services do not replace physicians, nurses, home health agencies, emergency services, licensed transportation providers, social workers, attorneys, financial advisors, or other regulated professionals.
Pricing is based on service needs and location. A consultation is required before services begin.
